   Andrew The vita-chamber was turned off in his office, and Vita Chambers work only if you die near them. He was not near them. It's also been said that they don't “revive” you, but merely make a clone of oneself. HOWEVER, the Vita Chamber in his office was turned off, yet you still spawn in it if you die. If you look at Bioshock 2, Eleanor takes DNA from over 10 years old and is able to recreate the main character. It's spotty in some places, but there's no real answer.
